- Want to submit your own Out-of-Network claim to your insurance provider? We make it simple! We will provide you with an invoice at the time of service which includes all of the necessary information (business name and address, tax ID, national provider identification, license numbers, etc… as well as the patient’s ICD-1 (diagnosis and CPT (billing) codes.

You will begin by providing the therapist with a detailed history of your problem. The therapist will take your input and ask you to perform a series of movements based upon the information provided to form an assessment of why you have an issue. From there, the therapist will devise a treatment for your specific issue, incorporating your goals and input for treatment.
This option is for physical therapy clients who have completed an initial evaluation. Based upon your treatment plan, the therapist will provide real-time, face-to-face instruction, feedback, and guidance on any therapeutic exercises and mobility tasks. The therapist will answer any questions you have regarding your treatment plan and assess your progress towards your goals.
For non-physical therapy specific treatment. Based upon your goals, the therapist will provide real-time, face-to-face instruction, feedback, and guidance on any general or pre-existing exercise plans. The therapist will answer any questions you have regarding general health and wellness, and assess your progress towards your goals.
